If something changed for you, we’d like to hear it.
When someone has spent years being told they’re fine while knowing they’re not, they don’t need another practice describing itself. They need to hear from someone who was in the same position. That’s something only you can give them.
How this works
You decide what happens to it.
Write it here, not on Google
A Google review is public and attached to your Google account. This isn’t. Write to us directly, and nothing you say leaves this practice unless you tell us it can.
Tell us what we may do with it
Three choices at the bottom of the form: share it with your first name and last initial, share it with no name at all, or keep it private and just for Dr. Chua. Any of the three is a good answer.
Change your mind whenever you like
We email you a copy of what you wrote and what you agreed to. If you ever want it taken down, one reply to that email is enough — no explanation needed.
If you’d rather post publicly on Google, that helps other people find us and we’re grateful for it — you can do that here. Google now lets you post under a display name of your choosing rather than your full account name, if you’d prefer that.
